This assignment requires you to select one patient scenario from the five available (see below) and to choose one of the identified priorities of care for the patient within the scenario. You are then required to provide an evidenced based discussion on how care will be implemented in relation to the chosen priority.

Provide a brief overview to demonstrate knowledge and understanding of the patient’s common health condition and how this may affect his/her physical, mental and social wellbeing (if appropriate)

Scenario 4 Scenario
James Cotter is a 55 year old divorced Scottish man, and attends a well man clinic following increasing headaches, some visual changes and pounding in his chest and head. He undergoes investigations in the General Practice setting for hypertension and elevated cholesterol levels. James has only had contact with health care for self-inflicted alcohol related injuries. He takes no medication, but is both a heavy smoker and heavy drinker of alcohol. He is very obese with a BMI of 31.2. He is very nervous of investigations and potential treatment, but is well known to the surgery and local hospital for treatment of minor injuries after alcohol-induced falls.
Following initial manual recording of blood pressure, the results of a 24 hour ambulatory blood pressure monitoring are: a B/P of 170/92 during the day and 130/83 at night. With a total cholesterol/ High density lipoprotein ratio of 7.5: Total cholesterol = 6.8, HDL= 0.9. Previous attempts to commence medication for health issues have proved difficult and it is a concern that James may be non-concordant.
Priority – Problems with eating and drinking: James has a very poor nutritional intake which consists mainly of McDonalds, Fish and Chips washed down by Cider or Beer; he consumes about 70 units per week
