Maria Leon is in her final year of a graduate program preparing to become a certified registered nurse anesthetist (CRNA). During the course of her graduate education, Maria observed that most people reported a burning sensation as propofol (a drug used to induce general anesthesia) was administered intravenously (IV). In conducting a review of the literature and discussing her observations with other CRNAs, Maria found several techniques used to minimize the injection pain. Based on this information, Maria decided that she would like to conduct a research study to examine the effectiveness of using lidocaine to reduce the injection pain of propofol. This project would fulfill the capstone requirement for her master’s degree.
A literature review of pain management led Maria to the gate control theory, which posits that there is a gating mechanism in the spinal cord. When pain impulses are transmitted from the periphery of the body by nerve fibers, the impulses travel to the dorsal horns of the spinal cord, specifically to the area of the cord called the substantia gelatinosa. According to the theory, when the gate is open, pain impulses ascend to the brain; when the gate is partially open, only some of the pain impulses can pass through. Pain medication has an effect on the gate, and if pain medication is administered before the onset of pain, it will help keep the gate closed, allowing fewer pain impulses to pass through.
In planning her research project, Maria used the gate control theory to guide the design and structure of the study. For the study, she decided to compare two techniques for pain prevention. One technique involved mixing 20 ml of a 1% propofol solution with 5 ml of a 2% lidocaine solution and injecting 1 ml of the mixture immediately before administration of the propofol. The second technique involved the placement of a tourniquet inflated to 50 mmHg on the arm in which the IV access device was placed. Then, 5 ml of 2% lidocaine would be injected and the tourniquet would be removed 1 minute later; propofol would then be injected. A time frame of 20 seconds would allow the clients to report pain in the arm before the propofol took effect. Maria also planned to have a control group that did not have either of the pain prevention interventions.
If the theory was correct, Maria hypothesized that both experimental groups would have less pain from the injection because the gate that allowed pain sensations would not open or would only partially open. She did not know which of the two experimental procedures would be more effective in preventing pain but was enthusiastic about conducting the study and adding to the body of knowledge on pain prevention in anesthesia.

Michael is a 22-year-old college senior whose GPA has declined with this semester’s grades.  Michael plans to apply to medical school and thinks that the lower GPA may prevent his acceptance to medical school.  For the last 2 weeks, Michael has skipped most classes because he has insomnia and fatigue.  Michael is now very depressed and has been thinking of suicide.  He took a loaded gun from his father’s gun cabinet and then wrote a suicide note to his family.  At the last moment, he telephoned 911 and told them of his suicide plan.  The police came, took the gun away, and then took Michael to the city hospital to be admitted for psychiatric treatment.   In the admission interview with the psychiatric nurse, Michael said that his pastor thought that only weak-willed people experienced depression and that it was a punishment for personal sins and the sins of one’s ancestors.  Michael told the nurse that he must be weak-willed and will never be able to accomplish anything.  The psychiatric nurse explained that multiple factors are the cause of depression.  The nurse told Michael that one theory holds that an imbalance of neurotransmitters, or chemical messengers of the brain, occurs in depression.  Neurotransmitters influence the individual’s emotions, thoughts, and subsequent behavior.  Recent research implies that neurobiology, heredity, as well as Psychological and environmental factors may be involved in the development and progression of depression.

Necrotizing Fasciitis Case Study

Introduction
C. S. is a 33-year-old, married, African American male who presented to the ED for progressively worsening body aches, abdominal pain, and swelling and draining in the peri-rectal and perineal area. Patient stated he “developed a pimple on his buttocks a week ago and it broke open today”. Patient also stated his “weakness and pain have been worsening over the past week”.
The only medical history consisted of hypertension and insulin dependent diabetes diagnosed four years ago, but patient reports he has not been taking insulin for at least one week. Patient is employed full-time and denies any family medical history, allergies, or alcohol, tobacco, or drug use. Patient was diagnosed with diabetic ketoacidosis (DKA) and peri-rectal abscess. Upon medical workup, patient was found to have necrotizing fasciitis / Fournier’s gangrene, so both infectious diseases and general surgeon were consulted.
Question 1: Explain the pathophysiology of necrotizing fasciitis? Give details about the cells involved and the process of inflammation. (4 points)

C. S. complains of fever and chills. He denies sore throat, nasal drainage or visual changes. He denies chest pain or palpitations. He denies any cough, wheezing or shortness of breath. C. S. does complain of abdominal pain, but denies any nausea, vomiting, diarrhea, black or bloody stools. He denies hematuria or dysuria. He also denies any rashes or easy bruising. C.S. does complain of swelling, pain, and redness in the perineal area. He denies any headache, syncope, near syncope or focal weakness. He denies any psychiatric or musculoskeletal problems. The remainder of a complete and careful review of systems is entirely negative and within normal limits.
Physical Exam
C. S. vital signs upon admission were: temperature 101.8, pulse 117, respirations 16, blood pressure 125/67, and oxygen saturation is 99% on room air. The patient’s general appearance is a well-developed, well-nourished African American male in no apparent distress. His head is normocephalic, atraumatic. Pupils are equal, round, and reactive to light. Extraocular motion is intact. There is no sclera icterus. Oropharynx is clear without lesion or exudates. Mucous membranes are moist. His neck is supple without mass, lymphadenopathy or JVD. His lungs are clear to auscultation bilaterally, without wheezes or crackles. His heart is regular, with tachycardia. No murmurs, gallops or rubs are noted. His abdomen is soft, nontender, with positive bowel sounds. His rectal/GU area revealed the presence of a left-sided peri-rectal wound with purulent drainage and confluent tenderness to palpation along the perineal area. His extremities are without clubbing, cyanosis, or edema. His skin is warm and dry, without rash or bruising. No neurological deficits are present. Patient moves all extremities. His speech is intact. Patient is awake and alert, somewhat flat affect, pleasant and cooperative.

Clinical Course
Upon receiving the CT results, C. S. was taken for emergent surgery for debridement of peri-anal abscess and multiple intravenous antibiotics were administered. Two days later the patient was taken back to surgery for another debridement and creation of transverse, loop colostomy since the abscess had spread into the rectal tissues. He was then treated with hyperbaric oxygen therapy. During his third treatment of hyperbaric oxygen, within the first 24 hours, he experienced a rapid increase in heart rate, but no shortness of breath or physical complaints. An EKG was performed after the treatment and he was found to be in supraventricular tachycardia. After cardiac consult and echocardiogram, patient was found to have a patent foramen ovale. Furthermore, after returning to his room on the unit the patient’s wife reported the patient having mental status changes. A CT of the head was performed and discovered a left thalamic lacunar infarct. A patent foramen ovale is the number one cause of stroke in patients under the age of 55. On the third day of admission C. S. did go into renal failure from acute tubular necrosis possibly from sepsis, hypotension, and/or antibiotics. Patient therefore was placed on hemodialysis.
