Discussion question
Community health centers were started to respond to the many health needs of some of the poorest and sickest communities in the nation. Centers were developed to serve the needs of communities beyond just medical care, but to provide resources for food access, housing, sanitation, and employment. Over time community health centers became known for understanding the populations they served and for offering superior primary care services. However, due to the rising costs of medical care, many community health centers veered away from public health interventions, health education, and environmental controls. Funding has been restricted to the delivery of personal medical services.
Using the weekly readings, the South University online library resources, and the Internet, research the role of community health centers in providing primary care services.
Based on your research, respond to the following:

  • Do you think it’s possible for community health centers to once again become more community-centered in their approach to population health with the goal of improving the health of communities?
  • What are some ways that they might do this?

Give reasons and examples in support of your responses. Be sure to cite any relevant resources.

He mentions to you, his co-worker, that he thinks he might have sprained his ankle over the weekend while playing soccer with some friends. He says it is swollen and very painful today and asks whether you think he should see a doctor or just wait for it to get better.
What might be some good recommendations for you to give Jeremy about his sports injury?

In any successful research endeavor, one of the first and most important steps is to define the study population. To survey or collect data on this population, researchers first must have access to individuals within it. However, access is not always guaranteed. In many cases, researchers must identify those in the position of power and seek their approval and buy-in of decisions related to a particular population. In the field of research, these individuals are known as gatekeepers. Crosby, DiClemente, and Salazar (2013) define gatekeepers as “people who are in positions to grant or deny access” (p. 107).
Consider the example scenario in this week’s Introduction regarding the reduction of waterborne illnesses. After the failure of the free shoe program, the organization now decides to investigate the feasibility of implementing a more hygienic set of latrines in a local school that serves hundreds of children. Who might be the gatekeepers in this situation? The school principal? The owners of the local water supply? Once these gatekeepers have been identified, what are the next steps in terms of gaining buy-in from them?
For this week’s Assignment, review the Learning Resources. Consider why certain individuals are gatekeepers and need to be involved in program planning and interventions.

You select either one of the emerging managed healthcare delivery models, Accountable Care Organizations or Patient-Centered Medical Homes, for your Final Paper. The 10-15 page paper (excluding title and reference pages) must follow APA style  and contain at least 10 scholarly, peer-reviewed, and/or other credible sources in addition to the course text.
Your paper must address the following bolded topics, which should be titled appropriately in your paper:

  • Include an Abstract which is a synopsis of the overall paper.
  • Managed Health Care Quality – Address what the selected emerging managed healthcare delivery model has done to improve quality of care.
  • Cost Containment – Describe how the selected model has striven to contain the costs.
  • Provider Contracting and Payments – Identify healthcare providers’ contracts and payment methods in the selected model.
  • Effects on Medicare and Medicaid – Summarize the impacts of the selected model on both Medicare and Medicaid.
  • The Emerging Role of Government Regulations – Examine the Patient Protection and Affordable Care Act (PPACA) policies in relation to the selected model.
