Your Concept Synthesis Paper is due this week. By the due date assigned, submit your scholarly paper, in which you have identified, described, researched and applied the concepts that underlie your personal philosophy for professional nursing practice.
Collate the work that you have done so far from Weeks 1–3, and submit your paper in the prescribed format to the Submissions Area.
Your paper should include:

  • Nursing Autobiography: A brief (1 page) discussion of your background in nursing.
  • The Four Metaparadigms: Identification, discussion, and documentation from the literature of your perspective on the four metaparadigms of patient, nurse, health, and environment.
  • Two Practice-Specific Concepts: Identification, discussion, and documentation from the literature of your perspective on at least two concepts specific to your own practice.
  • List of Propositions: A numbered list of at least five propositions or assumption statements that clearly connect the concepts described.

Your paper should integrate these discrete elements and reflect your personal nursing philosophy.
The paper should be thoroughly researched and well documented, with relevant material from the nursing theorists presented incorporated into the paper. The current APA Manual is to be used throughout the paper. Sources should focus on references from nursing theory but may also include conceptual and theoretical material from other professional domains. The paper, excluding references or appendices, is to be limited to 4-6 pages. Writing should be succinct and well organized, as it is impossible for the facilitator to evaluate form and content separately.

1.You are to research, analyze, and write an APA-formatted scholarly paper about the topic that you have selected.
2.Write an introduction that defines and describes the topic. Address what purpose the topic serves and how it impacts the delivery of healthcare in general, and nursing care in particular. Keep in mind that APA guidelines state you are not to use the heading of “Introduction,” but you should include it at the beginning of your paper.
3.Search for scholarly sources and relevant websites. Include a minimum of two scholarly sources. The course textbook does not qualify as a scholarly source. Cite all sources in the body of the paper and include them in the References list following proper APA formatting.
4.Provide one example of this topic. Describe the main features or aspects of the example with support from your sources.
5.Describe an experience where the topic impacted you personally, either when you were receiving health care, or when you were providing nursing care. Relate one positive aspect or one negative aspect of this experience and how it could have been improved.
6.Write a conclusion that summarizes the topic, purpose and how your newfound insight will influence your nursing care.
7.Use Microsoft Word to develop your paper. Use APA formatting. Refer to the Publication manual of the APA, 6th edition. Review the various APA documents included in this course and the Chamberlain Care Student Success Strategies (CCSSS) that can help you with your writing. Take advantage of the tutoring services that are available to Chamberlain students for free.

Question descriptionIn 500-750 words (not including the title page and reference page), apply a change model to the implementation plan. Include the following:

  1. Rogers’ diffusion of innovation theory is a particularly good theoretical framework to apply to an EBP project. However, students may also choose to use change models, such as Duck’s change curve model or the transtheoretical model of behavioral change. Other conceptual models presented such as a utilization model (Stetler’s model) and EBP models (the Iowa model and ARCC model) can also be used as a framework for applying your evidence-based intervention in clinical practice.
  2. Apply one of the above models and carry your implementation through each of the stages, phases, or steps identified in the chosen model.
  3. In addition, create a conceptual model of the project. Although you will not be submitting the conceptual model you design in Topic 5 with the narrative, the conceptual model should be placed in the appendices for the final paper.

Prepare this assignment according to the APA guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required.
