This is for Life Coaching, not a research project it needs to be inline as if you are a Life Coach.  Empowerment Coaching, Leadership Coaching, Health and Wellness Coach. Pretend you are the Coach and how you would do this. 
This is a two-part assignment.
Part 1-
Create an informed consent form, 500-750 words,that you might use in practice. Find some examples or templates from https://cirt.gcu.edu/?search=consent+form or http://www.who.int/rpc/research_ethics/informed_consent/en/.
Part 2 –
Find someone who is willing to have the informed consent applied. In 250-500 words, reflect on how the experience of going through the informed consent went:

  1. Describe what changes you would you make to your informed consent? What would you have to clarify more?
  2. What ethical guidelines influenced your informed consent?
  3. How did you describe the privacy/billing practices to them? Discuss what changes you might make for that portion of the discussion.
  4. What questions did the client have during the process? Describe how you addressed them?

Include three to four scholarly references.
You will turn in both your reflection and informed consent form, see “How to Turn In Multiple Documents” under the resources tab; click course resources.
Prepare this assignment according to the guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required.

In this assignment, you will gain an understanding of the crises that occur in development based on Erickson’s theory and utilize this understanding to apply this theory to your own development.
Erickson’s theory looks at development throughout life in a series of stages, and each stage identifies a crisis that occurs in life.
Based on your age, which stage of development does the theory predict you are currently facing? It is important to note that everyone’s development is unique and may not fall into this stage theory. You may feel that the previous stage or the next stage better explains your developmental process.
Write a 700- to 1,050-word paper discussing the stage of psychosocial development you are currently experiencing. Use the following questions to help direct you:

  • Where does the theory place you in the life stages?
  • Is this stage an accurate reflection of your current life stage? Do you believe that there is congruence between the developmental tasks and psychosocial crisis and your current life circumstances?
  • If so, briefly discuss the developmental tasks and the psychosocial crisis with which you are currently engaged. If not, what stage better addresses your life stage? What is the crisis being worked through in this stage?
  • Looking at your own career development and job satisfaction at your current or previous places of employment, analyze elements that have contributed to your current status based on your gender and cultural background.

Current theoretical perspectives on social movements provide an awareness of the nature of social movements and their role in defining human behavior. In terms of human services, it is important to be able to recognize and assess the costs and benefits to individuals who participate in social movement activities (for example, healthcare reform, charity organizations, and disability movements) as well as the effects of social movements on human behavior.
