How does the HCAHPS score affect the job description of nursing leaders?

Numerous vendor-designed and home-grown instruments have been used in the past in an attempt to capture patient satisfaction data. Unfortunately, the methods had little comparability between sites of care. The Centers for Medicare & Medicaid Services (CMS) and the Agency for Healthcare Research and Quality (AHRQ) jointly developed and tested the first national instrument and methodology to directly survey patients’ perception of care. The Consumer Assessment of Health Providers and Systems (CAHPS) was first implemented in the hospital setting on a voluntary basis in 2006 and then required as part of CMS’s Reporting Hospital Quality Data for Annual Payment Update program (RHQDAPU) in 2008. How would the use of the Roy Adaptation Model assist the nurse in planning the continuation of care for Sgt. Johns?

Part One: 

Neuman Systems Models is a flexible and adaptive model which can be applied to various aspects of nursing disciplines. Today, the model is used globally as a nursing conceptual model to guide curriculum development, research studies, and clinical practice in the full array of health-care disciplines.

Define a nursing practice area where you could apply Neuman Systems Model. What patient population would you target? How would you use this model to guide best practices? How would nursing outcomes be determined? Use at least one current research article related to your practice example.

Your initial posting should be at least 400 words in length and utilize at least one scholarly source other than the textbook.

Create a Power Point Presentation to discuss the following

Sgt. Eddie Johns leaned back against the chair in the outpatient orthopedic clinic. His head was killing him! He wasn’t sure which was worse, the “morning after” headache or not being able to sleep at night. At least when he had a few beers under his belt so he could catch a few hours of sleep. It had been like this since he was air evacuated back stateside from Afghanistan after the roadside bomb went off. He was thankful that he had only broken his leg in a couple of places and gotten a bad bump on the head. They called that traumatic brain injury but he didn’t know what that was and really didn’t believe them anyway. He was still thinking just fine. His friend Joe wasn’t so lucky! How was Joe going to learn to walk on those artificial legs? He was still in the hospital in Washington, DC. That was pretty far from his home. Eddie wished he could visit Joe. They had been in the same platoon for 9 months. But, Eddie was glad that he had been able to come back to his own home town for outpatient treatment. It took an hour to get here but at least he could see his girl almost every day. Sure he had moved back in with his mom but it was easier to have someone to help him get around and cook for him. It was a bummer that he couldn’t work right now. He guessed it didn’t matter much since he really didn’t have a job to go back to. He had been replaced at the plant. They said they would find something for him to do once he could get around more easily. Eddie sure hoped the doc would take the pins out of his leg today and give him a clearance to work.

  • How would the use of the Roy Adaptation Model assist the nurse in planning the continuation of care for Sgt. Johns?

  • Based on Neuman System’s Model, identify at least 4 stressors from the case study. Create a plan of care based on Neuman Systems Model for Sgt. Johns.

She is so excited about being in school finally, but her parents report that she is not participating in play time, seems weaker, and is pale. She is diagnosed with sickle cell anemia, parents undergo genetic testing as well as all her siblings and it is found that she is the only child with the disease.

Once the crisis is over what education will you provide to Roslyn and her parents?

Teaching Brochure Guidelines

PURPOSE

The purpose of this assignment is to allow students to integrate their knowledge of health assessment as it related to a medical diagnosis and teaching needs. Students will develop a tri-fold or bi-fold brochure for a client that will provide information about a specific disease, the related symptoms, and the management approaches.

COURSE OUTCOMES

This assignment enables the student to meet the following course outcomes:

CO2. Identify teaching/learning needs from the health history of an individual. (PO#2, PO#5). Explain expected client behaviors while differentiating between normal findings, variations, and abnormalities. (PO#1)

REQUIREMENTS

1. Research a disease process using two scientific articles from nursing journals.

2. Develop a teaching brochure which could be given to a client who is newly diagnosed with this disease to help them understand the signs and symptoms, disease process, and management approaches.

Sections to include:

· Introduction

· Subjective Findings

· Objective Findings

· Description of Disease (brief)

· Risk Factors/Causes

· Management

· Medications (Note: You have not taken your pharmacology course yet, so please do not focus on medication treatment. Under this section please add only one sentence “Your practitioner may recommend over the counter medications or prescription medications to treat your disease.” But, please do not go into any additional detail.)

· Conclusion

3. Include at least one image and up to three images that would help the client understand classic symptoms of the disease process. This picture should assist the class in better understanding the disease process.

4. Include your references on the back of the brochure in APA format.

5. Submit your brochure on time to the Dropbox (I will not accept emailed copies). Be sure to save the electronic file with your last name and first initial and the diagnosis you are describing.

PREPARING THE BROCHURE

1. Identify appropriate articles for review.

2. Begin to search for images in your textbook or online (make sure you reference your source).

3. Begin the project as soon as possible, allowing yourself plenty of time to make it look good. This means the brochure should be print ready. I expect these to look professional which means you need to spend some time on it.
