  8. I don’t think that health will never go down, since the cost of malpractice insurance is so expensive that a doctor has to have a certain income to cover that expense must their cost of living. The truth is that if the cost of health care won’t go down since its one of the biggest business in this country, in fact the cost does go down then what will happen to the economy; i have yet seen something like that in which the health care going down after years of being so expensive that would be the first. The fact that the over head expensive of health care won’t allow the cost to go down….that would be in a perfect world and that’s not the world we live in.

Given this scenario, do you think there is a point at which the cost of health care will not be able to increase? How will we know when we’ve hit that point?

Fixing the Health Care Plan Case Study

You are the Vice President of Compensation & Benefits for Wolfman Enterprises, a large, high-technology products manufacturer that employs approximately 65,000 in the United States.
You have just learned that health insurance premiums for the following fiscal year were expected to increase approximately 26%, up dramatically from the 16% increase of the previous year and the 14% increase the year before that.  Adding to your concern is the projection that, by 2015, the company’s $375 million annual health-care bill would increase to a staggering $613 million.
You have been asked by Wolfman’s top management to develop a custom-designed health insurance program for the organization that would hold down health-care premium costs to a reasonable level while ensuring that employees would retain adequate health care coverage.
The current health care plan is relatively traditional.  The company pays the entire health care premium for single employees (approximately $480 per month); dependent coverage is available, but the employee pays the entire cost of this coverage.
There is a $250 deductible, and no preferred provider network.  Once the employee’s deductible is met, the policy pays 80% of all covered expenses up to $5,000.  Once the $5,000 threshold is met, the policy then pays 100% of covered expenses up to a lifetime maximum of $2,000,000 per person.  Thus, each employee has an annual out-of-pocket maximum of $1,250.
Preventive care (an annual physical, routine Pap smears and annual mammograms for women, and PSA for men) are covered at 100% with no deductible.  Employees also have a prescription drug benefit.  After payment of a $100 annual deductible (per person), generic prescription drugs are available with a $10 copay; brand-name prescription drugs are available with a $25 copay.
Top management has asked you to provide them with a preliminary list of ideas to be considered, and how you believe employees would accept these changes, because the company is concerned about employee retention and does not want to lose valued employees.
Based upon the material presented in the textbook and your outside research, prepare a report that addresses what health care initiatives Wolfman might undertake in order to deal with the rapidly escalating health care costs.  Make certain that you consider the effects the initiatives would have on the employees and their acceptance of the changes.

Understanding how cultural beliefs and practices affect health and healing is an important aspect of being a health care provider.  Prior to responding to this discussion forum, answer the Cultural Self-Assessment Questions that are listed in Chapter 5 of your course text.  Based on your self-assessment, respond to the following:

Students will read and reflect upon the case study presented below.  Carefully read and review the information in the case study, and then reflect in a narrative upon the needs of the patient and the processes that must be in place surrounding this acute illness for a smooth and seamless transition for everyone involved.  Construct a voice-over PowerPoint presentation consisting of a minimum of 12 slides (including your title and reference slides) that will include the following information:
1.            Priority care needs of the patient at the center of the case study
2.            Priority care needs of the family care providers
3.            The disruption that this acute illness has caused for the patient, family members, and clinical microsystem
4.            The experiential features of acuity that are apparent in this case
5.            The elements that the clinical microsystem must possess in order to meet this patient’s care needs
6.            Potential barriers that may exist and must be managed in order to provide that care
7.            The communication strategies that were used in the delivery of care
8.            Well-defined (but flexible) roles within the clinical microsystem
9.            Potential strategies to plan for unexpected changes in care needs
10.          The Deployment Flowchart (see page 225, Nelson text) which must include:
•             Evidence-based core measure algorithms
•             Structured decision making and standing orders
•             Advanced access
